Residents in Independent Living are urged to take the survey online starting on October 2 and ending October 29. Those who take the survey during the first week will enter an early bird raffle which gives them the opportunity to win four chef’s dinner tickets. Prizes will be awarded during the following three weeks too.

At the end of the survey you will receive a confirmation number. Write that down and show it to someone at the front desks. The number will not be recorded; it shows you completed the survey and are eligible to enter the drawing. You will receive a raffle entry form to complete. It will be entered into the drawing.
